Deep Laser Skin Resurfacing is a treatment to reduce deep facial wrinkles produced by age and sun exposure.  The procedure combines the benefits of the Micro Laser Peel and the Profractional Laser.  These laser are set to maximally target the deep papillary dermis with the result of replacing surface sun damaged skin and wrinkles with more youthful, vibrant, and smoother skin.  The procedure can be used to target the whole face or smaller areas like around the mouth and eyes.

The Micro Laser Peel is a ablative laser that is used to remove a sheet of superficial epidermal skin which is then replaced with fresh skin with more youthful properties.  Unlike many other laser systems, the built-in technology allows us to carefully control the depth of ablation from 10 microns ( equivalent to a deep microdermabrasion ) to 1000 microns ( a deep skin resurfacing )

Patients should not think of their Resurfacing Treatment as their final treatment.  Additional lighter treatments may be beneficial in producing incremental improvements.  Also, we strongly recommend that you maintain your results with yearly Triple Laser Package Treatments and and BBL treatments at 3 month intervals

Skin resurfacing with a laser at Aesthetica Cosmetic Surgery & Laser Center is a noninvasive aesthetic treatment that utilizes pulsating beams of energy to eliminate dead and damaged skin cell layers. The procedure is used for addressing facial wrinkles and lines and for minimizing skin flaws including pigmentation, acne scars, and other issues. Sometimes known as a laser peel, fractional skin resurfacing is considered an ideal procedure for people who want clearer, softer, younger-looking skin. Based on the needs of the patient’s skin type, the treatment may be customized for best results. There is little recovery time required for most laser skin resurfacing facials. Any questions or concerns may be reviewed with one of our experienced registered nurses during your assessment and consultation at our Leesburg office.

Aesthetica Cosmetic Surgery & Laser Center offers four kinds of laser resurfacing skin procedures that are selected depending on your skin goals. Some are performed for milder cases of skin damage, while others are more aggressive:

  • Ablative Laser Treatments
    Ablative lasers treat thin skin layers, similar to dermabrasion or a deep chemical peel, with a high level of control that permits the laser technician to reduce damage to the skin. Ablative lasers are ideal for firming the skin as well as treating acne scars and wrinkles.
  • Non-Ablative Laser Treatments
    Non-ablative laser treatments don’t remove the skin — the laser light simply makes micro-injuries to the skin cells to encourage natural skin healing. Non-ablative facials are great for addressing wrinkles and fine lines on those who are willing to wait 2 – 3 months to see strong results.
  • Fractional Laser Treatments
    Fractional laser treatments are a combination of non-ablative and ablative that make small holes in the deeper skin layers. The body’s self-healing process leads to tighter, firmer, and healthier skin. Fractional laser treatments can also be performed to treat fine lines, sun damage, acne scars, and pigmentation issues.
  • Micro Laser Peel Treatments
    The micro laser peel is an ablative laser that is used to remove a sheet of superficial epidermal skin which is then replaced with fresh skin with more youthful properties. Unlike many other laser systems, the built-in technology allows us to carefully control the depth of ablation from ten microns (equivalent to a deep microdermabrasion) to 1000 microns (a deep skin resurfacing). This treatment is ideal for minimal to moderate fine lines and wrinkles, hyperpigmentation, and sun spots.

You have multiple factors to think about when selecting which laser peel procedure is best for your needs. Laser skin resurfacing is ideal for people who are unsatisfied with their skin’s appearance. If you see skin damaged by the sun, age spots, acne scars, wrinkles and fine lines, pigmentation, or redness troubles, a laser peel procedure may be helpful to improve these complexion blemishes. Also, a laser peel is wonderful for those who have rough texture, scaly patches, or other skin surface problems. A laser peel isn’t suggested for anyone who has very dark skin tones or active acne outbreaks. Additionally, it’s important to know that there is a recovery period needed after laser treatments. Certain patients, based on their skin condition, may require as much as 2 – 3 weeks for their skin to normalize.

How is a Deep Laser Resurfacing performed?

Deep Laser Skin Resurfacing combines the benefits of the Micro Laser Peel and the Profractional Laser 

  • Step 1 – Oral sedation is provided in selected patients
  • Step 2 – Topical numbing cream or numbing shots are given depending on the depth of treatment
  • Step 3 – Your eyes will be protected with safety goggles
  • Step 4 – Laser Peel is performed first and takes about 30 minutes
  • Step 5 – Profractional Laser is performed next and takes about 30 minutes
  • Step 6 –  Protective ointment is applied to the skin

Most patients will return to work in 2-3 weeks depending on the depth of treatment

Before you are scheduled for a more intense or deeper laser peel treatment, you may be given a retinoid product for 2 – 3 weeks before your laser peel. The retinoid helps prep the skin for more effective results.

Medication may be suggested prior to the procedure to help lessen any pain for increased comfort throughout your appointment. If your complexion only needs mild improvement, a local anesthesia may be applied to numb treated areas of the face.

For the whole complexion or deeper laser procedures, a kind of sedation may be required. Throughout the procedure, you can relax in our special treatment chair while the laser handpiece passes over your skin, directly targeting the needed surfaces for rejuvenation.

You may feel the heat of the laser followed with cold air from the system’s built-in cooling device. The laser energy is designed to remove damaged and dead skin cells while also prompting new collagen production.

The recovery after a Laser Skin Resurfacing can be variable depending upon the depth of treatment

  • Expect some areas of bleeding and the appearance of road rash
  • Most patients will return to work in 2-3 weeks depending on the depth of treatment
  • Apply post laser ointment generously for 3-5 days
  • Apply vinegar soaks after 3 days
  • Oral narcotic pain medication is prescribed but is not often needed
  • Redness may be present for 4-8 weeks but can be camouflaged easily with make-up
  • Final appearance is not apparent for 2-3 months

Following light peels, an ointment will be smoothed over the treated skin and you can go home for your recovery. The treated skin may look pink and there may be some swelling and irritation. These symptoms should subside in a few days and the skin peeling process will occur over a 3 – 5 day period. For those who are getting a deeper laser peel, your recovery time will take longer. In addition, the treatment sections may be protected with dressings and the burning, irritation, and swelling symptoms may be stronger. Pain medication can be prescribed to lessen these symptoms. The recovery period for a more intense laser treatment may last several weeks. Every patient needs to minimize their UV exposure following their laser peel and use sun protection throughout the day. For all laser peel treatments, the treated skin will naturally crust, flake, and peel off. It’s crucial to avoid accelerating the recovery process to prevent scarring and to shield your skin from environmental exposure. After the peeling is finished, the treated skin should feel smoother while also appearing rejuvenated, clearer, and younger.

As with any procedure, there are associated risks.  Only you can decide whether risks for you outweigh the many benefits.  These can include but are not limited to:

  • Abnormal scarring
  • Irregular pigmentation changes
  • Undercorrection
  • Infection including the triggering of cold sores
  • Prolonged erythema
  • Delayed healing

What’s the best type of resurfacing for me?
One of our registered nurses will help you decide which laser resurfacing treatment is best for your needs and goals during your consultation. The best treatment for you will depend on the health and condition of your skin, your skin concerns, and how deep you want your treatment to go. How long you are willing to give your recovery may also be a deciding factor when choosing your treatment type. When in doubt, it’s best to start with a lighter treatment to see how you like your results and adjust as needed for your next treatment.

How can I help my skin recover after a resurfacing treatment?
A member of our team will talk to you about how you can help your skin heal after a laser resurfacing treatment. The most important rule is don’t pick or peel your skin. This could cause lasting damage including scars. You should skip all hair removal methods in the treatment area, which means no shaving, waxing, or depilatory creams. We will recommend an ointment to help soothe and heal your skin.

Should I get laser skin resurfacing or microneedling?
Depending on your skin concerns and goals, one of our registered nurses will help you decide if laser skin resurfacing or microneedling treatment is best for you. While they all improve the surface of the skin, each has its own benefits and drawbacks. During your consultation, each option will be explained and recommendations will be made for your treatment.

How often should I get laser resurfacing treatments?
Depending on the resurfacing type you and Dr. Chang select for your treatment, you may need to wait weeks or even months between appointments. Deeper treatments will provide longer-lasting results, but will require a longer recovery period. You should talk to Dr. Chang about how often you are willing to come back for treatments so he can take this into account when recommending a type of resurfacing during your consultation.

How quickly can I return to work after a laser treatment?

2-3 weeks

How is a Micro Laser Peel different than a Deep Laser Resurfacing?

They are admittedly very similar.  The treatment and results lay on a spectrum.  They are both systems on the Sciton Laser Platform and produce a controlled sheet of laser ablation.  The Micro Laser Peel is defined as a peel up to 200 microns deep.  A Deep Laser Resurfacing produces the same energy but goes deeper than 200 microns.  We will sometimes do a deep resurfacing around the mouth at depths up to 1000 microns while going much lighter on the cheeks.  This is the beauty of the Sciton Laser Platform.  We can control which laser we want to use and at different depths depending on your individual needs.  

Why would you combine a Micro Laser Peel with a Profractional Laser?

The combination of a Micro Laser Peel with a Profractional Laser Peel gives us significantly more control of how and what we want to treat.  The micro laser peel produces a sheet of ablation which is ideal for improving skin texture and fine lines; but if the treatment is performed  too deeply, it will take longer to recover.  The Profractional Laser Peel produces columns of ablation instead of a continuous sheet.  This allows us to get into the deep dermis which in turn allows for deep collagen production and contraction.  The uninjured skin around the columns help heal the skin faster.  

And if you add a Forever Young BBL to the combination, we call this a Triple Laser Package.  The Forever Young adds the additional benefit of improving skin pigmentation and the potential for rejuvenating cellular DNA

What is the Forever Young BBL?

The Forever Young BBL uses broad band light to reduce the appearance of brown sun and age spots in addition to rejuvenating cellular DNA.  The end result is more vibrant and youthful skin with more uniform pigmentation.  Unlike older lasers, this technology can treat virtually any area on the body, including the face, neck, chest, arms and hands. 

What is the Micro Laser Peel

The Micro Laser Peel is a ablative laser is used to remove superficial epidermal skin which is replaced with fresh cells with more youthful properties.  Unlike many other laser systems, the built-in technology allows us to carefully control the depth of ablation from 10 microns ( equivalent to a deep microdermabrasion ) to 1000 microns ( a deep skin resurfacing ).  The laser can be used to:

  • Smooth skin
  • Remove UV damage
  • Smooth fine lines and wrinkles
  • Treat skin growths like tags, moles, and keratosis
  • Remove superficial disorganized cells and replace them with healthy collagen

What is the Profractional Peel

The Profractional Laser Peel is used to reduce fine lines and tighten skin by producing more collagen.  It is a cosmetic skin laser that creates clean, micro-ablated channels in the skin, while leaving the surrounding tissue uninjured.  The channels created by the laser energy fill in with new collagen while the intact tissue promotes rapid healing due to the presence of stem cells and melanocytes in the papillary dermis.  

What is Complete Skin Resurfacing

Complete Skin Resurfacing does exactly as the name implies.  The ablative laser technology allows for the removal of deeply aged and wrinkled skin and replace it with fresh more youthful skin.  This is accomplished by combining the Micro Laser Peel and the Profractional Laser at deeper setting that have the ability to strongly resurface the skin but to also induce skin contraction.  This process can be directed toward the whole face or can be more targeted to areas around the eyes or mouth.  Upper lip lines is a perfect target.  

What is the Triple Laser Peel ( most popular ) –  This is the most popular laser combination that is performed in our office.  The triple laser combines the benefits of the Profractional Laser, the Micro Laser Peel, and the Profractional Laser.  Each of these are tunable and can be set to different depths and strengths.  Younger patients with less damage will need a laser combination at lighter settings.  Older patients with more damage will benefit from our pro or plus settings.  Together, you will have more vibrant and youthful skin as will as the anti-aging benefits of the Profractional Laser; and smoother and tighter skin with less wrinkles from the Micro Laser Peel and the Profractional Laser.
